When this template is required

Required of every California healthcare employer — hospitals, skilled nursing, behavioral health, home health, EMS, drug treatment programs, correctional healthcare. Effective since April 2017, more rigorous than SB 553 (which is general industry). Hospitals must report workplace violence incidents to Cal/OSHA via the Workplace Violent Incident Reporting System.

Unit-specific plans required

T8 §3342 requires WVPP elements specific to each unit, service, or operation. Generic plans get cited. The template includes the unit-specific plan framework so each unit can complete its own.

Patient-specific risk factor framework

Subsection 3342(c)(10) requires procedures to identify patient-specific risk factors — psychiatric condition, substance use, history of violence, confusion/disorientation. The framework documents the assessment process while respecting privacy.

Workplace Violent Incident Reporting System

Hospitals (GACH, APH, SH) must report incidents to Cal/OSHA via the Workplace Violent Incident Reporting System within 24-72 hours depending on severity. The plan includes the reporting procedure and crosswalk.

SB 553 (Labor Code 6401.9, effective July 2024) applies to all California employers generally. T8 §3342 (effective April 2017) applies specifically to healthcare. The healthcare standard is more rigorous: unit-specific plans, patient-specific risk factor assessment, hospital-specific reporting to Cal/OSHA. Healthcare employers are subject to T8 §3342 specifically (and not to SB 553's general requirements for the healthcare context covered by §3342).

What's the Workplace Violent Incident Reporting System?

Cal/OSHA's online reporting system for hospitals. General Acute Care Hospitals (GACH), Acute Psychiatric Hospitals (APH), and Special Hospitals (SH) must report each violent incident to the system within 24-72 hours depending on severity. The reporting includes incident type, classification, location, employee impact, and response. The plan documents the reporting procedure.

How does this integrate with the IIPP?

T8 §3203 (IIPP) and §3342 (Healthcare WVPP) both apply to CA healthcare employers. The §3342 plan can be incorporated into the IIPP or maintained separately. Most healthcare employers maintain them as separate documents that cross-reference. The template includes the integration cross-reference.

Do non-hospital healthcare employers need this?

Yes — the standard applies broadly to healthcare. Skilled nursing, home health, hospice, EMS, behavioral health, outpatient — all are covered. Some elements (like hospital incident reporting) are hospital-specific, but the core WVPP framework applies to all covered healthcare facilities.
